The Accursed are humans who for one reason or another have monstrous qualities. These are the tradeoffs for playing a special snowflake: giving up some control of what you will look like, and suffering a punishing downside. Accursed can be thieves, magic users, or fighters. Never clerics. Roll three mutations. Roll one upside and one downside. Mutations are meant to have little impact on mechanics but they should not be ignored. Players of an Accursed still have control over their character's appearance as normal, but should keep to the spirit of what they rolled. Accursed are assumed to look like ordinary humans except where the mutations have corrupted their forms.
In the eastern parts of Quao, Accursed are not treated much differently differently than ordinary people. In Nifoland and Nidomland they are respected over ordinary people. But in western Quao and most other places they face some level of mistrust.
